CHARLESTON, W.Va. (The Hinton News) – Governor Patrick Morrisey announced a $1.25 million funding opportunity through the State’s Rural Health Transformation Program (RHTP) to support the development of the Program of All-Inclusive Care for the Elderly (PACE), an all-in-one care model that coordinates medical care and daily support to help older West Virginians with serious health challenges stay safely in their own homes. Released under the Smart Care Catalyst initiative, this funding will help local organizations launch and run these comprehensive networks to expand access to coordinated, community-based care.
“We are committed to helping older West Virginians live with dignity, comfort, and independence in the communities they love,” said Governor Morrisey. “This opportunity is designed to back local organizations that are exploring easy, community-based ways to care for our neighbors.”
This opportunity will fund the startup and expansion of daytime health and activity centers, serving as the first step toward a complete, coordinated care system. These day centers will offer a warm, structured environment where older West Virginians can access clinical care and daily support in one place. Services will include professional nursing care, help managing medications, coordination of doctor visits, and enriching social activities, allowing those who face daily health challenges to get the care they need without leaving their communities.
The broader goal of this initiative is to build local networks that help older West Virginians stay independent, healthy, and out of nursing homes. By coordinating care and offering everyday health services close to home, these programs are designed to prevent sudden health emergencies, cut down on unnecessary hospital visits, and help older adults manage chronic conditions.
“Older West Virginians facing major health issues often have to juggle multiple doctors, medications, and daily tasks, which can be incredibly stressful in rural areas,” said Secretary of Health Dr. Arvin Singh. “This grant will fund coordinated programs that bring doctors, therapies, and social activities together, making it easier for older adults to live safely at home and giving their family caregivers much-needed peace of mind.”
